Terms and Conditions

Parent Code Of Conduct
The Ute Conference has adopted the following Parent Code of Conduct (the “Code”) for the 2025 season. The Code and its requirements are a reflection of the Ute Conference’s goals, values, and beliefs relative to the proper role of parents in supporting their child in sports. Prior to registering his or her child to participate in Ute Conference activities, each parents should read the Code. In signing his or her child up to play within the Ute Conference, each parent expressly agrees to abide by the Code and acknowledges that they or their child may be subject to discipline or sanctions if they fail to comply.

 

Any parent who violates the Code at any Ute Conference game, practice, pep rally, or other activity will be asked to leave the field or place at which the activity is taking place and will also be suspended for up to three (3) additional games. That parent’s player will also be suspended for one (1) full game following the violation. If a parent refuses to leave upon being asked, or does not abide by a suspension imposed by the Ute Conference, the parent will be subject to heavier sanctions, which the Ute Conference may impose in its discretion. Such sanctions include but are not limited to (i) longer suspensions for both the player and parent, (ii) the parent’s player being disqualified for the remainder of the season, (iii) forfeiture of past and future games, and (iv) the parent’s player’s team being disqualified from participating in playoffs. Repeat violations of this Code may, in the Ute Conference’s discretion, lead to greater sanctions, including without limitation full-season suspensions. Violations of the Code will be determined by the Ute Conference executive board in its sole discretion. All determinations are final. Parents do not have any rights to appeal or request reconsideration.

 

In allowing his or her son or daughter to participate in Ute Conference activities, each parent agrees as follows:

 

1.          I understand I am not a coach. Therefore, I agree to cheer on my player without interfering with instructions given by the coach.

 

2.          I understand I am not an official or referee. Therefore, I agree to let the officials/referee(s) call the game without interference. I understand and acknowledge that referees and officials may, from time to time, make calls that I don’t agree with.


3.          I understand I am not a player. Therefore, I will never enter the field of play without being invited by (i) the head coach of my player’s team, (ii) a district board member, (iii) a Ute Conference executive board member, or (iv) a referee officiating my child’s game.


4.         I understand and agree that, first and foremost, children participate in the Ute Conference to have fun, and I agree that Ute Conferences games are for the benefit of the children who participate in those games, not their parents, other adults, or the coaches.


5.         I (and any family members who attend to support my child) shall not and will not engage in any kind of unsportsmanlike conduct towards any official, referee, coach, player, or other parent. Unsportsmanlike conduct includes but is not limited to booing, taunting, using profane language or gestures, threats, unwanted or uninvited physical contact, fighting, entering the field of play, going onto either team’s sideline, any conduct inconsistent with the Ute Conference’s Bylaws, or any form of conduct that constitutes a criminal offense under Utah law.


6.         I will never ridicule or yell at my child, other players, or any coach for making a strategic error, performing poorly, accidentally committing a penalty, or losing a game.


7.         I will respect the officials and referees and their authority during games. I will never directly question, belittle, criticize, or confront a referee or official before, during, or after a game. I acknowledge, understand, and agree that it is impossible for any official or referee to call a perfect game and that each official or referee, in every game they preside over, will make mistakes. I agree that watching from the stands or the sidelines is not the same as watching and monitoring the game on the field. I agree that shouting abusive, profane, or critical words at an official or referee does not positively contribute to the game nor improve the quality of the officiating.


8.         If I wish to calmly discuss any issue that does not pertain to player safety with a coach, I agree to call or email the coach to make arrangements for a face-to-face discussion. No coach is required to listen to or entertain complaints via telephone, email, or text message. I agree not to address complaints or concerns (other than those pertaining directly to player safety) immediately following a game or practice.


9.         If I have a concern that I am unable to resolve with my player’s coach (after following the requirements of paragraph 8 above) I will contact the appropriate person within the Ute Conference Football organization.


10.         I will demand and provide a sporting environment for my child that is free from illegal drugs, tobacco, and alcohol, and I will refrain from their use at all Ute Conference events.


11.         I will do my best to ensure that my child attends all games and practices. When attendance is not possible, I agree to inform my child’s coach(es) in advance.


12.         I understand and acknowledge that coaches (i) are non-paid volunteers and (ii) are not babysitters, daycare, or chauffeurs. I agree that I am fully responsible for my child’s conduct during Ute Conference practices, games, pep rallies, and other activities.


13.         I agree to control my non-playing children for their own safety and comfort of others, including by keeping my non-playing children off of the field of play. I understand and agree that I am also responsible to ensure that my non-playing children abide by the Code and all other Ute Conference Rules. If one of my non-playing children violates the Code or other Ute Conference Rules, I acknowledge and agree that both and I my participant child will be subject to discipline, in the Ute Conference’s sole discretion.

 

I agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary action.

Online Registration Only –
Registration through the Ute Conference is done online. There is not an in-person option.

Electronic Signature Agreement
Electronic Signature – In electronically signing your player’s registration paperwork, you consent to all the Bylaws, Policies, Game Rules, and Terms and Conditions of the Ute Conference. The Ute Conference has the right to prohibit from participating or being involved with Ute Conference activities any person, player, or family member who does not comply with the above-stated standards.
All completed registration forms will be stored in the Ute Conference database archives.

 

District Boundaries – The Ute Conference strictly adheres to a “play where you live” standard. That standard is articulated in full at Section 15 of the Ute Conference’s Program Rules. Consistent with that standard, I/we understand that the Ute Conference District boundaries coincide with the High School boundaries in which my son or daughter resides, except as otherwise delineated on the official Ute Conference District map. I/we acknowledge that I/we am responsible for verifying that my child is registered and plays within the District in which my child permanently resides. Penalties for playing out of the appropriate Ute Conference District can be imposed on an entire team, and they include but are not limited to (i) player suspension and disqualification, (ii) forfeiture of any games a wrongfully-registered player participated in, and/or (iii) possible elimination from playoffs for an entire team.

Equipment – Parents are fully responsible returning all Ute Conference equipment issued to their children. All equipment will be returned in the same condition in which it was issued, normal wear and tear excepted, by the deadline set by either the Ute Conference or by the District in which the player plays. Any player who fails to return all Ute Conference-issued equipment by or before December 15 of the current season/year will be assessed a fine of $350.00. The player will not be able to participate in subsequent Ute Conference activities until the fine is paid in full. The Ute Conference reserves the right, and shall have absolute discretion, to assign or sell collection authority to an outside agency for immediate collection of outstanding equipment fines. If the Ute Conference sends a fine to collections, the applicable player’s parents will be liable for the full amount of the fine, plus interest at the statutory rate, costs of collection, and reasonable attorney fees. A player who quits prior to the end of a season will have two (2) weeks from the date on which they quit to return all Ute-Conference issued equipment. After the two-week deadline, a $350.00 fee will be assessed and sent to a third-party collection agency.

Redshirt Rule for Juan Diego – A player who played the previous season in the Juan Diego
District may not register to play in the district in which he/she resides until they have “redshirted” (not participated) for one season. Conversely, a player who played the previous season for the district in which they reside must redshirt for one season before they may register in or play for the Juan Diego district. Notwithstanding the foregoing, if a player physically changes permanent residences such that he or she resides in a district that differs from the one in which he or she played the previous season, he or she may register for the following season in either (i) the district in which he or she now resides or (ii) the Juan Diego district.

Age Requirement – Parents agree and represent that their son or daughter is in fact the actual age as represented in their registration paperwork and understand that their eligibility to participate is determined by his/her age. The age of each player on September 1 of the current year determines age classification eligibility.

Coach Placement – Parents understand and agree that the coaches are selected and determined at the district level. The Ute Conference conducts criminal background checks and enforces its background check policies

Team Placement – Parents understand and agree that the team their child participates on and the coach they play for is determined solely by the team coaches and District Officers unless special circumstances require intervention on the Conference level.

Insurance Coverage – The Ute Conference provides catastrophic medical insurance coverage for injuries sustained while participating in any sanctioned practice or games. Coverage becomes primary for uninsured players. There is a deductible ($250.00 for 2025), which parents/guardians must pay, and the coverage has a maximum payout per accident. All claims must be submitted by the parent/guardian to the Ute Conference’s insurer no later than sixty (60) days after the date of the accident. For additional information concerning the submission of a claim, a parent/guardian may contact the Ute Conference Executive Board.

Registration Insurance – ($35.00 per player – only available at time of initial registration.) At registration, parents may purchase registration insurance, which affords coverage for the non-refundable registration fee parents pay when registering their child for Ute Conference activities. In the event that injury, illness, or other covered peril, as defined below, prevents the child from participating, the parent can file a claim with the Ute Conference seeking reimbursement of their registration cost.

A player’s parent or guardian goes on active military duty, necessitating a move
Involuntary job transfer necessitating a move


Insurance coverage reimburses for registration fees less hard costs (i.e. uniform), prorated by the remaining days left in the season. To illustrate, if a player is injured on the first day of practice, his parents/guardians will be reimbursed the entire registration fee, less a prorated deduction for a single day of the season. By contrast, if the same player is injured during the final week of the season, his parents/guardians will receive a much smaller reimbursement, namely the prorated registration fee for the lone remaining week of the season.

Liability Release and Indemnification – As consideration for my son or daughter (“Player”) being permitted to participate in Ute Conference activities, I hereby waive, release, and forever discharge the Ute Conference, Inc., their districts, sponsors, organizers, officers, executive board members, team coaches, supervisors, assistants, and those transporting participants to and from any Ute Conference activity (together, the “Releasees”) from any and all claims, demands, causes of action, damages, risks, hazards, injuries, and personal injuries to my son or daughter that arise from, accrue as a result of, or are caused by his or her participation in any Ute Conference activity. This release is intended to release and discharge in advance all claims, causes of action, demands, liabilities, and damages arising from or sounding in negligence, carelessness, or any unintentional tort, but not including intentional torts or intentionally-caused injury. I understand that the above activity involves one or more elements of danger, the risk of injury, and/or death. I hereby assume those risks for and on behalf of my minor child, whether such risks are currently known or unknown to me. Furthermore, I voluntarily clearly and unequivocally agree to assume those risks, with the realization that I have the alternative of choosing not to have my child participate in this activity. It is further understood and agreed that this waiver, release and assumption of risk is to be binding on myself, my heirs, my agents, my spouse, executors, administrators, and assigns. I, Player’s parent, shall defend, indemnify, and hold harmless Ute Conference and the Releasees against any and all losses, damages, liabilities, deficiencies, claims, actions, judgments, settlements, interest, awards, penalties, fines, costs, or expenses of whatever kind, including attorney fees, fees, the costs of enforcing any right to indemnification under this Agreement, and the cost of pursuing any insurance providers, incurred by the Ute Conference or Releasees, arising out of or resulting from any claim of a third party (including but not limited to a former spouse who is Player’s other biological parent) related to participation in Ute Conference activities, including but not limited to (i) any claim related to my or Player’s own negligence or (ii) the ordinary negligence of Ute Conference or the Releasees.

VIDEO, PHOTOGRAPHY AND SOCIAL MEDIA RELEASE – By participating or allowing my son or daughter to participate in any Ute Conference activity, I hereby understand, acknowledge, consent, and agree that the Ute Conference may, in its sole discretion, use all photographs, videos, illustrations, and/or digital images taken or captured by the Ute Conference or its agents at, during, and/or while travelling to or from Ute Conference activities for the Ute Conference’s promotional or advertising purposes, including without limitation on social media sites and in other digital or print media. I, my agents, executors, heirs, representatives, beneficiaries, administrators, trustees, and assigns, hereby waive, release, remise, and forever discharge the Ute Conference, Inc., its affiliates, agents, representatives, districts, sponsors, organizers, officers, board members, employees, team coaches, supervisors, assistants, and all those individuals transporting participants to and from Ute Conference activities from any and all royalties, damages, risks, liabilities, claims, demands, harm, or injury that arise from or relate to the Ute Conference’s use of such photographs, videos, illustrations, or digital images. I understand and agree that all photographs, videos, illustrations, and digital images described in this paragraph are owned exclusively by the Ute Conference. Further, I understand and agree that neither I, my spouse, my former spouse, nor my son or daughter are entitled to any fees, payments, royalties, or wages for appearing in any Ute Conference promotional, advertising, or social media materials or postings. I hereby agree to assume all risks associated with my son or daughter appearing in any Ute Conference promotional or advertising materials or postings and acknowledge that I have the alternative of choosing not to participate or have my child participate in a Ute Conference activity. I, Player’s parent, shall defend, indemnify, and hold harmless Ute Conference and the Releasees against any and all claims, demands, causes of action, liabilities, lawsuits, or actions arising out of or resulting any claim of a third-party (including my former spouse who is my son’s or daughter’s biological parent) related to the Ute Conference’s use of such photographs, videos, illustrations, or digital images.

Children’s Online Privacy Protection Act (COPPA) Privacy Notice – Your player(s) information has been entered into Ute Conference, Inc. website. The information collected is directly from the parent/guardian. The personal information provided is for team rostering and administration of the player. No information will be disclosed to third parties; any business and or mailing lists. The operators of the Ute Conference’s website will not require more information than is reasonably necessary for the participation in Ute Conference activities. The parent/guardian can review the player(s) personal information at any time.

Consent for Medical Treatment – I hereby give my consent to have the my son or daughter receive first aid and medical treatment or to be treated by a physician or surgeon in case of accident or injury sustained while participating in a Ute Conference activity. I understand I am responsible for paying the deductible ($250.00) associated with coverage under the Ute Conference’s current insurance plan. I further understand that costs of medical care, which exceed the limit of applicable coverage, will be borne at my sole expense. The location of the activity and/or the nature of the illness or injury may require the use of emergency medical personnel, and I authorize the Ute Conference to contact and engage emergency medical personnel for care if the Ute Conference, in its discretion, deems such care is necessary.

Refund Policy for Registration Fees – Prior to the first day of practice, a player may receive a full refund of all registration fees (less a $35.00 administration fee), regardless of the player’s reason for withdrawing from the Ute Conference. A refund will not be issued unless and until (i) the player has returned all equipment the Ute Conference issued to him or her and (ii) the returned equipment is in substantially the same condition as it was when issued. Beginning (and including) the first day of practice, a player shall not receive any refund unless (i) they purchased Registration Insurance, as described above, and (ii) they qualify for coverage in accordance with the Registration Insurance terms and conditions above. There are no exceptions to this provision. To receive a refund, a parent/guardian must provide a copy of their registration receipt to the Ute Conference.

Sales Policy – All sales of merchandise from districts are final and non-refundable.

Website Usage – The Ute Conference’s website uses cookies through the WordPress web content management system. By using the Ute Conference’s website, parents/guardians agree to the Ute Conference’s use of cookies, as outlined in WordPress’s cookie policy.

Communication Preferences – Each player parent consents to receive SMS messages (including text messages), and telephone calls (including prerecorded and artificial voice and autodialed) from the Ute Conference, its agents, representatives, affiliates, or anyone communicating on our behalf at the specific phone number(s) you have provided to us, concerning Ute Conference activities. Each parent, upon registering his or her daughter or son, certifies, warrants, and represents that the telephone number they have provided to the Ute Conference is their correct contact number and not someone else’s phone number. They further represent that they are permitted to receive calls and text messages at the telephone number they have provided to Ute Conference. Additionally, each parent consents to receive email communications through the email address provided with their completed registration. Each parent agrees to promptly alert Ute Conference whenever they change or stop using the telephone number and/or email address they provided to the Ute Conference. The Ute Conference, its agents, representatives, affiliates, and anyone calling on the Ute Conference’s behalf may use such means of communication described in this section even if a parent will incur costs to receive such phone messages, text messages, e-mails, or other means. Standard message and data rates may apply to all SMS messages (including text messages). Additionally, each parent consents to receive email communications through the email address provided with your completed registration.

Are you passionate about supporting youth sports and making a positive impact on the community? We are currently seeking a 1099 Contractor who has experience in running youth sports organizations. Seeking someone who is passionate at assisting in moving the UC forward with our existing team.

Note: UCR’s, Presidents, and Board Members of current districts along with current coaches in the UC will not be considered. Past presidents, coaches and board members will be considered.

Qualifications:

– Minimum of 3 years of youth sports experience.
– Strong communication and interpersonal skills.
– Ability to work independently and collaboratively in a team.

Types of working projects and requirements:

1. Social Media Management:
   – Develop and maintain social media presence.
   – Create engaging content to promote events, achievements, and community involvement.

2. Database Management:
   – Handle data entry and maintenance of the organization’s database.
   – Ensure accurate and up-to-date information for players, coaches, and volunteers.

3. Spreadsheet Expertise:
   – Proficient in Excel for tracking and analyzing various data points.
   – Assist in creating and managing game schedules and platforms, assist in tournaments and referee placement.

4. Graphic Design Skills:
   – Utilize Adobe Illustrator for designing promotional materials, banners, and graphics.
   – Collaborate with the marketing team to enhance visual content.
   – Create and collaborate in the uniform design process.

5. Basic Accounting Knowledge:
   – Support the finance team with basic accounting tasks.
   – Assist in budget tracking, and financial reporting.

Requirements:

– Strong organizational and multitasking skills.
– Proficiency in social media platforms and content creation.
– Experience with database management and data entry.
– Advanced skills in Microsoft Excel for spreadsheet management.
– Familiarity with Adobe Illustrator for graphic design.
– Basic accounting knowledge is a plus.

Media Team Application

The UC has 12 AI cameras that require camera techs to operate. These cameras are not the typical sport cameras and provide 4K broadcasting quality recordings. Minimum requirements for this position are:

  • At least 18 years of age
  • Reliable transportation
  • Great communication skills including the ability to talk to strangers
  • Must pass a background check and UC Photographer/Videographer Certification course
 

Duties:

  • Set up, start, and monitor AI Cameras on Saturdays during the 2023 UC Season.
  • Troubleshoot cameras if technical problems arise.
  • Deliver media to the UC via web upload

 

Compensation: $18/hr

Parent Code Of Conduct
We, the Ute Conference, have implemented the following Parent Code of Conduct Agreement for the important message it holds about the proper role of parents in supporting their child in sports. Parents should read, understand this form prior to their child participating in our program.

Any parent guilty of improper conduct at any game or practice will be asked to leave the facility and be suspended for the remainder of that game and three (3) additional games. Your player will also be suspended for one (1) full game following the incident. Repeat violations may cause season ending suspensions. I therefore agree:

My role as a parent is as follows:
1. I understand I am not a coach. Therefore I agree to cheer on my player without interfering with instructions given by the coach.
2. I understand I am not an official. Therefore I agree to let the official(s)/referee(s) call the game without interference. Understanding that they may make some calls that I don't agree with.
3. I understand I am not a player. Therefore I will never enter the field of play without being invited by the head coach or district official(s).

Parents, coaches, players, and referees may only act in a singular role during a game. They may not act in dual roles (i.e. An individual acting as both a coach and a referee; An individual acting as both a parent and a coach; etc...)

I also agree to abide by the following:
1. I will remember that children participate to have fun and that the games are for the youth, not adults.
2. I will remember that “wins” are based on my child’s performance, teamwork, and playing within the rules.
3. I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing, taunting, or using profane language or gestures.
4. I will never ridicule or yell at my child or other participants for making a mistake or losing a game.
5. I will respect officials and their authority during games and will never question, discuss, or confront officials or coaches at the field. I understand that it is impossible for any referee to catch everything that goes on during such a fast-paced game as football. Watching from the stands or the bench is not the same as watching and following the game on the field. Shouting abuse at officials does not contribute to the game, it only communicates to those around us that we need to prove to others how much we know about the game by belittling the official. During the season everyone should expect some mistakes made by players, coaches and officials during games.
6. If I wish to calmly discuss the actions of the coach, I will wait 24-hours before calling the coach to make arrangements for a face-to-face discussion. No coach is required to listen to complaints over the phone or directly following a game or practice.
7. If I have a concern that I am unable to resolve with our coach (after observing the 24-hour rule) I will contact the appropriate person within the Ute Conference Football organization.
8. I will demand a sports environment for my child that is free from drugs, tobacco, and alcohol and I will refrain from their use at all sports events.
9. I will insure that my child will attend all games and practices. When attendance is not possible, I agree to inform the coach or team manager in advance.
10. I will remember coaches are not babysitters. Parents are ultimately responsible for the actions of their child while at the field or any other team function.
11. I will control non-playing children for their own safety and comfort of others.
12. Conduct Subject to Discipline:
A. Making physical contact with any player, coach, official, league representative, field personnel or spectator.
B. Taunting or threatening any player, coach, official, league representative, football personnel or spectator.
C. Using profane and/or vulgar language or mannerisms.
F. Going onto the playing field.
G. Being involved in any activity that would warrant the summoning of law enforcement officials.
H. Any other conduct that is not in compliance with the bylaws of the Ute Conference Football program.

I agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary action as stated above.

Electronic Signature Agreement
Electronic Signature – Registration Form and Receipt will be completed at the end of the signup process by virtue of and electronic signature. You hereby consent to all the Bylaws, Policies Terms and Conditions of The Ute Conference, Inc. Understanding that prior to completion you have every right to withdraw and sign up in person. A $25.00 in person registration fee may be assessed by the Districts. All completed registration forms will be stored in the Ute Conference database archives. Completed Electronic Signature will appear as signed on the registration form.

For Players Ages 7 – 14 years old (age on July 1, current year)

District Boundaries – I/we understand that the Ute Conference District boundaries coincide with the High School boundaries in which my son or daughter resides. I/we acknowledge that I/we am responsible for verifying that he/she is registered within said Conference districts. Penalties for playing out of the appropriate Conference district can be imposed on an entire team including forfeiture of any games a wrongfully registered player participated in and possible elimination from playoffs for an entire team.

Equipment– We agree to be fully responsible for the return of the items of equipment issued to the above named player. All equipment will be returned in a reasonable condition, with only normal wear and tear when requested to do so by District or Conference Officer or pay the replacement value of $350.00. No equipment will be accepted after December 31st of current season/year. If the equipment is not returned by December 31st collection responsibilities will be turned over to an outside agency for immediate collection of the stated value of $350.00, plus all costs of collection, including a reasonable attorney fee plus interest. A player quitting prior to the end of the season will have 2 weeks to return equipment to District Officer or it will be turned over to an outside agency for immediate collection actions as stated.

Red Shirt – Be advised that a player previously registered to play in the Juan Diego District may not register to play in the district in which he/she resides until they have “red shirted” (not participated) for one season. The same rule applies to a player who has been playing in the district they currently reside in and wishes to register in the Juan Diego district. This “RED SHIRT” rule can be suspended only in the event of a player physically moving residency from one district to another prior to changing registration as previously stated.

Age Requirement – I/we further represent that my son or daughter is in fact the actual age as represented above and understand that their eligibility to participate is determined by his/her age. The age of your player on September 1, of current year determines age classification elgibility

Team Placement – I/we also understand that the team my child participates on and the coach they play for is determined solely by the team coaches and District Officers unless special circumstances require intervention on the Conference level.

Insurance Coverage – I/We understand that the Ute Conference, Inc. provides catastrophic medical insurance coverage for injuries sustained while participating in any sanctioned practice or games. Coverage becomes primary for uninsured players. There is a deductible ($250.00) and maximum payout per accident. All claims must be submitted no later than 60 days after the date of the accident.

Liability Release – In consideration of the acceptance of my application for participation in the above activity, I hereby waive, release, and forever discharge the Ute Conference, Inc., their districts, sponsors, organizers, officers, team coaches, supervisors, assistants, and those transporting participants to and from the above-described activity from any and all damages, risks, hazards, and personal injury to my child named above as participant incidental to the above described activity which may hereafter result or accrue as a result of participation in said activity. This release is intended to release and discharge in advance those based on negligence or carelessness, but not for intentional torts or intentionally causing injury. I understand that the above activity involves an element of danger and risk of injury. I hereby assume those risks, known or unknown to me. Furthermore, I voluntarily clearly and unequivocally agree to assume those risks, with the realization that I have the alternative of choosing not to have my child participate in this activity. It is further understood and agreed that this waiver, release and assumption of risk is to be binding on myself, my heirs, executors, administrators and assigns.

VIDEO, PHOTOGRAPHY AND SOCIAL MEDIA RELEASE – By participating or allowing my son or daughter to participate in any of the activities contemplated or described herein, I hereby understand, acknowledge, consent, and agree that the Ute Conference may, in its sole discretion, use all photographs, videos, illustrations, and/or digital images taken or captured by the Ute Conference or its agents at, during, and/or while travelling to or from Ute Conference activities for the Ute Conference’s promotional or advertising purposes, including without limitation on social media sites and in other digital or print media. I, my agents, executors, heirs, representatives, beneficiaries, administrators, trustees, and assigns, hereby waive, release, remise, and forever discharge the Ute Conference, Inc., its affiliates, agents, representatives, districts, sponsors, organizers, officers, team coaches, supervisors, assistants, and all those individuals transporting participants to and from the above-described activities from any and all damages, risks, liabilities, claims, harm, or injury that arise from or relate to the Ute Conference’s use of such photographs, videos, illustrations, or digital images. I understand and agree that all photographs, videos, illustrations, and digital images described in this paragraph are owned exclusively by the Ute Conference. Further, I understand and agree that neither I, my spouse, nor my son or daughter are entitled to any fees, payments, royalties, or wages for appearing in any Ute Conference promotional, advertising, or social media materials or postings. I hereby agree to assume all risks associated with appearing in any Ute Conference promotional or advertising materials or postings and acknowledge that I have the alternative of choosing not to participate or have my child participate in a Ute Conference activity.

Children’s Online Privacy Protection Act (COPPA) Privacy Notice – Your player(s) information has been entered into Ute Conference, Inc. website. The information collected is directly from the parent/guardian. The personal information provided is for team rostering and administration of the player. No information will be disclosed to third parties; any business and or mailing lists. The operators of this website will not require more information than is reasonably necessary for the participation in our football league. The parent/guardian can review the player(s) personal information at any time.

Consent for Medical Treatment – I hereby give my consent to have the above participant receive first aid and medical treatment or to be treated by a physician or surgeon in case of accident or injury while participating in the above activity. I understand I am responsible for the deductible ($250.00) associated with coverage under the current insurance plan. I further understand that costs of medical care, which exceed the limit of applicable coverage, will be at my expense. The location of the activity and/or the nature of the illness or injury may require the use of emergency medical personnel.

Refund Policy – No exceptions will be made for this policy: Prior to 1st day of practice, a full refund less the $25 fee (Online, Administration and Organizational Costs) will be refunded. No refund will be given after the first day of practice as defined in the Ute Conference Bylaws. Refunds will only be issued after all equipment has been returned to the district. You must provide a copy of your registration receipt in order to receive a Refund.

Sales Policy – All Sales of merchandise from districts are Final and Non Refundable.

Communication Preferences- You consent to receive SMS messages (including text messages), and telephone calls (including prerecorded and artificial voice and autodialed) from us, our agents, representatives, affiliates, or anyone communicating on our behalf at the specific number(s) you have provided to us, with service-related information, questions about your use of the Services, and/or Even Account and marketing information. You certify, warrant, and represent that the telephone number you have provided to us is your contact number and not someone else’s. You represent that you are permitted to receive calls and text messages at the telephone number you have provided to us. You agree to promptly alert us whenever you stop using a telephone number. Even and our agents, representatives, affiliates, and anyone calling on our behalf may use such means of communication described in this section even if you will incur costs to receive such phone messages, text messages, e-mails, or other means. Standard message and data rates may apply to all SMS messages (including text messages). We may modify or terminate our SMS messaging services from time to time, for any reason, and without notice, including the right to terminate SMS messaging with or without notice, without liability to you. Additionally you consent to receive email communications through the email address provided with your completed registration.
